About the UPMC Session
Communication tools supporting information exchange between health care providers are woefully lacking, especially those tools intended to support primary care to post-acute care (PAC) transitions. EHR systems enable providers to generate referrals, which are mainly “open-loop” requests that typically don’t include tools to ensure that referrals are successfully completed.

UPMC combined applications currently in place with SES’ clinical data exchange platform to integrate PAC homecare consults from the ambulatory EHR with their centralized intake department to transform a manual referral workflow into an automated closed-loop process. UPMC created a cost-effective PAC referral process that was seamless, efficient, and scalable in a large integrated delivery and finance system.

Healthcare organizations can realize cost savings by using interoperable solutions to streamline patient communications and increase provider collaboration. This presentation will demonstrate the business impact of leveraging the investment in IT solutions currently in place rather than building or buying new ones.
