Partnership to improve health information exchange for geriatric and long term care providers.
July 9, 2015 – Rockville, MD – Secure Exchange Solutions has partnered with Eldermark Software to introduce seamlessly integrated Direct secure communications into the state-of-the-art Eldermark Senior Care Software platform as well as Eldermark Exchange™, the leading health information exchange (HIE) for senior living providers, ancillary providers and resident physicians.
The Secure Exchange HISP and Direct exchange products are integrated into many leading electronic health records (EHRs) representing over 70 speciality areas. In the long term care segment, IT leaders choose Secure Exchange to help its customers maximize their visibility and become preferred referral partners for community hospitals and physicians, as well as to improve clinical and administrative information exchange, including transitions of care, referrals and prior authorizations.
Eldermark Senior Care Software has over 1,200 long term care facility customers leveraging ten comprehensive software modules that combine to provide a complete solution that includes health information exchange, nurse call, billing, point of care charting, risk management and more. With the addition of Direct messaging through Secure Exchange, long term care facilities can now receive transitions of care and referrals from hospitals directly into their Eldermark platform.
Eldermark Exchange™, with its mission of improving outcomes, reducing healthcare costs and improving public health, will now use Direct secure messaging as a channel to access patient data, creating the complete longitudinal records which are known to reduce unnecessary testing and hospital readmission rates.
“Secure Exchange allows us to offer our customers the greatest connectivity to hospitals and physicians nationwide,” said Craig Patnode, president of Eldermark. “Direct secure messaging, integrated into the Eldermark platform, helps our customers ensure that complete patient records accompany the patient when they first arrive in a long term care facility.”
“Long term care and other geriatric providers are the cornerstone of healthcare for older adults,” said Dan Kazzaz, CEO of Secure Exchange. “Connecting these providers into the greater healthcare community is crucial for better transitions and optimized referrals.”
